How we do Cardiopulmonary Exercise Test.

CPET is conducted as a maximal exercise test, performed on a stationary bike. Here is typical steps:

  1. Preparation: Before the test, patients are advised to avoid heavy meals, caffeine, and smoking for 3 hours. Comfortable clothing and shoes suitable for exercise are recommended. An individual is fitted with a mask connected to a machine that measures the volume and gas concentrations of inhaled and exhaled air. This setup allows for the analysis of oxygen consumption and carbon dioxide production.
  2. Incremental Exercise Protocol: The test begins at a low intensity, which is gradually increased in a stepwise manner. This can involve increasing the resistance on a stationary bike.
  3. Monitoring: Throughout the test, heart rate, blood pressure, and perceived exertion are monitored. The individual is encouraged to continue until they reach their maximum effort. In some cases, we can monitor lactate level as well.
  4. Reaching VO2 Max: The test continues until the individual can no longer maintain the required intensity, or until oxygen consumption plateaus despite an increase in workload. This plateau indicates that VO2 max has been reached.
  5. Data Analysis: The data collected is analyzed to determine the maximum volume of oxygen the individual can utilize during exercise, which is expressed as VO2 max in milliliters of oxygen per kilogram of body weight per minute (ml/kg/min).

CPET/VO2 max testing provides valuable insights into an individual’s aerobic fitness and is used in both clinical and athletic settings to assess cardiovascular and respiratory efficiency.
